Fra pochi giorni a Chicago si terrà il meeting annuale dell'American Society of Clinical Oncology; gli abstract sono già disponibili e i linfomi sono ben rappresentati: faccio un sunto di quelli che mi paiono più interessanti.

LNH grandi cellule B - Lo studio finale del GELA sul raffronto fra R-CHOP14 e R-CHOP21 nel trattamento dei pazienti anziani con linfoma grandi cellule B (DLBCL) conclude che non c'è differenza fra i due trattamenti (Conclusions: Results of the final analysis of LNH03-6B demonstrate similar efficacy and safety profile between R-CHOP14 and R-CHOP21)

LNH follicolare - Lo studio finale della FIL (Fondazione Italiana Linfomi) sul raffronto fra R-CVP, R-CHOP e R-FM nel trattamento del linfoma follicolare avanzato (FL) conclude che il TTF a 3 anni è inferiore per i pazienti trattati con R-CVP rispetto alle altre due terapie, mentre OS a 3 anni è simile (98%, 95% and 93% rispettivamente per R-CVP, R-CHOP e R-FM); il gruppo trattato con R-FM ha mostrato una maggiore incidenza di tumori secondari (Conclusions: This trial showed that R-CVP was associated with an inferior 3-year TTF and PFS compared with R-FM and R-CHOP. OS was similar among study arms but R-FM showed a higher rate of secondary tumors).

LNH follicolare - Uno studio americano di fase III ha paragonato R-CHOP con CHOP e I-131 Tositumomab (RIT-CHOP) nel trattamento del linfoma follicolare avanzato (FL) concludendo che i risultati sono molto simili. (Conclusions: Both regimens produced outstanding PFS and OS, and no statistically significant differences between them were observed.)

LNH follicolare - Lo studio americano di II fase conclude che lenalidomide + rituximab sono più attivi della sola lenalidomide nella cura di pazienti con FL ricorrente, a fronte di tossicità simili (Conclusions: Lenalidomide + rituximab is more active than lenalidomide alone in patients with recurrent FL with similar toxicity. A trend toward lower thrombosis risk with LR may relate to greater anti-tumor efficacy. The LR regimen warrants further study in FL including as a backbone for addition of novel agents in relapsed and frontline settings.)

LNH aggressivo - Lo studio prospettico tedesco esamina pazienti con LNH aggressivo cellule B o T refrattari, con ricaduta precoce (<12 mesi) o recidivi dopo autotrapianto. I pazienti hanno ricevuto un condizionamento mieloablativo seguito da trapianto allogenico da donatori consanguinei o no. Lo studio conclude che il trattamento ad alte dosi seguito da trapianto allogenico mostra eccellenti risultati in pazienti pretrattati refrattari o con ricaduta precoce. Conclusions: Lymphoma-debulking (high-dose) chemotherapy followed by alloSCT shows excellent results in heavily pretreated patients with early relapse or primary refractory aggressive lymphoma. There was evidence of graft-versus-lymphoma activity in this setting. For patients with a fully matched (10/10) related or unrelated donor the results compare favorably to autoSCT or alloSCT following reduced-intensity conditioning.

LNH grandi cellule B - Lo studio finale tedesco esamina il ruolo della radioterapia di consolidamento nei pazienti anziani con DLCBL e malattia bulky. Lo studio conclude che i pazienti anziani in remissione completa (CR) dopo 6xR-CHOP-14 + 2R non traggono beneficio dalla radioterapia sulle masse bulky, al contrario di quelli che non raggiungono la remissione completa. Conclusions: In the R era, radiotherapy to bulky disease does not improve the outcome of elderly pts. in CR/CRu after completion of R-CHOP-14 immunochemotherapy, but appears to be beneficial for pts. with Bx not achieving CR/Cru. By restricting Bx radiotherapy to patients not achieving a CR/CRu, 43% of the patients with Bx could be spared radiotherapy

LNH grandi cellule B - Lo studio giapponese multicentro di fase II esamina pazienti con LNH aggressivo cell. B (DLCBL compreso) refrattari o recidivi dopo R-CHOP, trattati con bendamustina + rituximab (B-R). Lo studio conclude che questa è una terapia di salvataggio promettente. Conclusions: B-R demonstrated promising activity in pts with relapsed/refractory DLBCL. Toxicity was primarily hematologic and generally manageable. These results suggest that B-R is a promising salvage regimen for pts with relapsed/refractory DLBCL after R-CHOP.
